ONLINE VOLUNTEER PROFILE FORM

Thank you for filling out our online Volunteer Profile Application form. Please take the time now to print out the last section of the application and either mail it to us or you can fax it. If you have not yet filled out the Profile form, you can do so below.

This last section is to provide us with autorizations. You will have to sign:

  • To authorize the Distress Centre of Durham Region to contact the references you provide
  • To authorize investigation of all statements in your application and releasing Distress Centre of Durham Region and all others from liability in connection with same.
